Labels are printed in zip code order and are not returnable. Only Avery 5162 pressure-sensitive labels are available. A $5.00 shipping and handling fee is applied to all requests. We do not provide diskettes or tapes.

Orders must be placed in writing and accompanied by an order form and payment. Labels will be provided for one time use only. They may not be reproduced in any form or resold to another user.

Please allow 2 weeks for delivery from the time the order and payment are received.

The mailing piece must be of professional interest to AUPHA members and subscribers. A sample of the mailing piece must accompany each order. A draft is acceptable if the printed piece is not available. AUPHA must be included in the mailing. AUPHA reserves the right to refuse any order.
